Are these like a bag? or are they a flat sheet? Also, do they come with elastic to help secure to the headrest? Also, what plastic is it? Thanks.

  They usually fold over at one end and have like a 30 cm "pillow slip" thing to slide over the headrest. Loose sheet from about shoulder-height down.
