Email include details of my LinkedIn, which I excluded for privacy reasons. Appears legitimate to me.

May be premium users only.

Options included either Myer gift card, Paypal or donation to charity.

EDIT: Upon completing the survey for $10 paypal, I was only asked for a name and email.

  I get these about once a quarter, but for $20 - so usually do them! Why not!
  It's for LinkedIn Learning specifically, in my situation.

  I got one for $5
