It's very weird, I can't find Gift Voucher this app on my Lumia 925, and I sign in to Windowsphone.com, it saids, this app is not available for your phone. (I am sure my Lumia is AU stock.)

Interesting. Their Terms and conditions page as Ariz mentions below includes 925 in the list; the main page does not. Must be a mistake. I Wonder which is the real one.
I picked up a brand new 925 today from Allphones, same problem. The T&C clearly state the 925 is included.
Who do we contact to sort this out?

This is contrary to the T&C, I guess they will fix it soon.

$40 Credit for my 1020 on plan from Telstra since December.
Also I activated the code and my account balance is now $80 not $40!? Not sure if error or what but my balance shows $80. Will have to wait and see what happens.

Can confirm that my 1020 bought from HN last October was not eligible for this particular promotion. I did however claim a $20 voucher a few months back which may have affected this claim.

I purchased Lumia 1520 on 30 Apr 14 from Harvey Horman but still not able to redeem. I am getting "This promotion is for selected Lumia models only, purchased during the promotion period determined in the Terms and Conditions". Am I doing anything wrong? Do I need to register my phone first with Nokia.

only need a MS account and you should be good to go I believe. Thats all it seemed to need for me. You can have a Nokia account through your MS account, so you don't need to make a separate one.
Edit the headline, This does not work for ALL LUMIA owners. I have a 720 and app will not download. Edit your headline to selected Lumia models

Sigh worked for a relative's 625 but not for my 1020, and i purchased mine later.
Maybe I already registered my phone to my windows acc? or maybe because i registered a nokia acc? oh well, its not like i was entitled to this.
Confirmed working with Lumia 1520 brought from eglobal ( grey import ) in late January :)
did not work for my 1020…i bought mine second hand off someone who had purchased from HN last year (so maybe the reason it didn't work is they might have already claimed the earlier $20 voucher).
