It has 5 buttons (scroll wheel is middle + 2 near the thumb, made for righties). Scroll wheel does up/down, left/right.

I was able to snag one of these at the Bing Lee at Haymarket. Their RRP is slightly inflated, but 15$ is still less than the cheapest I could find on shopbot.
