Its a pretty big call that this app claims to do it, when years of complaining from Apple users hasnt changed Apple's restrictions on this issue.
All you need to do to create a custom ringtone on iOS is to take a 30 second snippet of a song, save it as an mp3 and change the extension to *.m4r and add it into itunes. I'm assuming this app does something similar to this but in a more automated manner.

it works and it works great on my wife's iPhone 4s and My iPhone 5 for text and ringtone
Pretty sure it was iOS4 (possibly 5) that enabled this. I was waiting for years for this to happen! .m4r can now be used for Alarms, messages… ie. Any alert tone.
Also: On my stock iPhone 5 you can set any tone as a text tone, so what this app does seems perfectly "allowable" by Apple's standards.
How? I've been using iPhones for a number of years now and using iPhone 4 at the moment and can't setup customer ringtone for SMS's. Same with my wife's 3GS.
Unless this is possible in iOS 6 which i haven't upgraded to yet.
