Unless you are changing ownership structure or have something weird going on, no.
No. No one refinances their mortgage by themselves.
nah fairly easy just talk to bank. cut the broker as well. take matter in your hands.

Should be easy without one. Your new bank will arrange the finance approval then you just need to complete the discharge form from your old bank (some banks have them are online, for others you may need to go into a branch), can take around 3 weeks for them to arrange the discharge as they wont be in a rush to lose you. They will probably try to keep you as a customer too so that may be a good time to negotiate if you are interested.

The idea it 'hits your credit score' is a joke, the bank may flag you for a closer look but of your making payments you'll be fine.
You do it yourself. There is a fee for the land title office but you don't need to do anything, the bank pays it on your behalf.
