I purchased the vouchers so can state with confidence there is no name on the voucher, and it could be used by anyone. You do need to specify the bowling alley you want to go to - that's printed on the voucher, but beyond that, no problems.
Voucher valid for 1 time use within 3 months from date of issue. Not valid with other offers. Valid on day of redemption only and any unused value is forfeited. Vouchers are non-refundable, not exchangeable for cash and cannot be resold or transferred. Lost or stolen vouchers cannot be refunded. This voucher is redeemable at AMF Bowling Centres in Australia only. Full terms and conditions are listed on amfbowling.com.au and can be changed at any time without notice.

Interestingly, no. Damn! I will not be happy if shoe hire is indeed extra.
Edit: It's not explicitly excluded, just that it's not included within the Terms and Conditions at https://shop.amfbowling.com.au/Page/terms
On a logical basis, I would assume shoes are included. If you look at their regular prices, they sell "games" and those games include shoe hire. But yeah, it would be better if it was explicitly included in the terms and conditions.

In Bangkok recently I went bowling - the chicks at the service counter were 9.9/10 - and wore very revealing kit….
Anyway they charged for shoe hire separately from bowling - so you paid $x per game of bowling and $y for shoe hire (not charged per game)….

normally when you buy a game you buy for a lane dont you? and it doesnt matter how many plays. this per person per game would make it not that great a deal if you want to go with a few people. or am i ubderstandibg it wrong?

Even though you are hiring a lane you actually pay for every person that bowls.
So with this promotion it would cost you $20.00 for 4 people to bowl one game.
The reason behind this being that the more people that bowl on your lane, the longer you will be using that lane for.
No, you always pay per person, not per lane.
