Are additional card holders 16-digit number the same as the account holders number?

Nah, you as the primary account holder are responsible for purchases from additional card holders so you are the only one that needs to undergo a credit check

I thought they always had a new number and I believe most banks are different. CBA is the only bank I have encountered that gives you the exact same number as the account holder.
So is Westpac, which is bit of a pain. If a card is faulty they issue another with the same number and hope that the new card works. If one card is lost or stolen, then both cards need to be reissued.
The statement will differentiate which card is used by the different number.

Edit: statements show the different numbers too, so you can work out which card was used for which purchase.
