Bupa has both emergency only cover and premium cover with also covers non-emergencies up to $5,000 Australia wide.
I could be wrong, but Bupa also is Aus wide which is good (except for states where you don't need cover), many others I checked are only for the home state.
There are reciprocal agreements between the states. I'm with Ambulance Victoria but if I need an ambulance in another state then it's covered, as long as I use that state's official ambulance service and not a private one. If I receive a bill afterwards, I can just forward it to Ambulance Victoria and they will take care of it. This is explained in the Frequently Asked Question section of their website.

Needed an ambulance in QLD near the border earlier this year and NSWAS attended. Sent me a bill for like a grand. No no no, Qld address, transported to Qld hospital. Quick email and away it went. Shouldn’t have even been sent in the first place.

Nothing is free, it just qld extra tax cover their ambulance cost.
Of course it’s not free, but it shouldn’t cost the user per use, it should be fully subsidised in all states, paid for through taxes.
The way it’s set up in NSW there are exemptions already for when it’s reasonable or not to have have the user pay eg mental health. People shouldn’t be getting into debt if they have an accident or a heart attack. Or weighing up whether to go to hospital or just leave something due to the cost. Even incurring a debt because someone else calls an ambulance for you. We’re meant to have access to fee free (yes it costs the gov/tax payer) essential public health care in Australia, surely ambulances should be part of that.
The extra it would cost to make it fully subsidised in NSW would be easily covered by gambling tax revenue - better than it being siphoned off back into clubs for ‘community development’, ie lining their mates pockets.
QLD used to have a subscription scheme, but later made it fully subsidised.
