Absolutely. If you used a private certifier then you need to apply/send the form to them. Ask them for the form and what's required.
If you are being certified through council then ask council for the form and what you need.

They don’t do it.
First of all, do you even have a Construction Certificate? If you don't, then you dont need an Occupation Certificate.
I was told by builder I need it. This is for an addition.
You dont get an Occupation Certificate without a Construction Certificate.
Then I must have one. The builder likely took care of that for us.
@Mahalo: If your builder has it, then your builder should be preparing the Occupation Certificate for you however that is not common practice.
Did you have a DA or CDC approved by Council for this second storey addition?

Only a Certifier / Council Building Surveyor can issue a Construction Certificate for a DA.

When I did renos my certifier applied for the occupation certificate. I had to provide documents such as waterproofing, termite treatment, construction certificate.

Our builder wanted to charge $450+gst. It's literally just filling in a 2 page PDF and organising the certification reports (which the builder should give you). DIY for sure
