Neuron Mobility will help you get to and from your Covid-19 vaccination appointment by giving out two $5 coupons to each Neuron rider who completes this form until all the coupons are fully allocated.
It is on a first-come-first-serve basis, so don’t wait!
All you need is to have a Neuron account and your appointment details. After we receive your form, the coupon code will be sent to your email.
*If no coupon is found under the "Rewards" section in the Neuron app within 4 hours, this signifies that the coupons have been fully allocated.
