I'm trying to find something to implement at our factory. We want people to have to declare each day when they walk in. It should be fairly simple - scan a QR, goto a website, add name, phone, email, and say that you agree to the terms, confirm you aren't sick or near someone who is sick.
The NSW government one actually isn't a declaration, it's just an I AM HERE in essence.
If i build one from Google Forms, it doesn't actually take specific action if you select results which should alert someone (I AM SICK)
A lot of the ones i can find online are the same, they don't actually flag (even visually) that you've given a bad answer. They just say "thanks for submitting" regardless
We really want it to direct you to a screen saying "You can't enter, please leave" or something for the security guard to identify, otherwise that person is already on our site.
Does anyone have any experience with this?? I can't believe it's so difficult to find one!

Best I can think of is a Google Form that is set up like this: https://www.howtogeek.com/437542/how-to-create-a-google-form…. Selecting Yes for being unwell or being near someone unwell would direct them to a page that asks them to take an action (leave, contact a supervisor, etc). If they select No it would submit as normal.
IIRC you could add an image or large coloured text to those pages for easy identification by staff when the screen is shown.
