which size should I go for?
Small is similar footprint to 13" laptop, Medium is similar to 15" laptop. Large is fairly big for many workspaces. Id be going medium if you will be using it mostly full time at a desk with an external monitor or larger laptop. If you are travelling heaps, have limited desk space and/or no external monitor the small will be excellent and do all you need.
it depends on how you like to draw. If the way you are going to draw from your arm, choose a bigger one. If drawing from the wrist, a smaller one.
Just a note, they are the old models. There was a pretty small iteration update earlier this year. What Ive noticed - went from RF wireless (usb dongle kit) to BT wireless built in, slightly different design and USB C instead of Micro USB. Well worth a purchase if these features are not deal breakers. Luckily/unluckily for me I can't really afford these prices right now so Ill be saving up for the new one anyway!

Can anyone confirm if the intuos pen will connect to an iPad for note taking and drawing?
No it won't work. Only works with the compatable model of graphics tablet.
