The Gentos(Japanese) Headlamp has excellent reviews both on BCF and SCA. Plus seems like a great price for what it is.
One of the reviews:

  Brought the Gentos headlamp last time it was on special ($23.90) and while I've only used it a few times (camping + cave visit in NZ) - it does the job well.
  Bottom line, very good price for what it is.

  I bought one that looked exactly like the $6.49 one a while back. It was a piece of junk and i threw it out.

    I purchased a few of those $6.49 headlamps years ago, but from Anaconda. A couple are reasonable for what they are but the others are terrible. Quality varies enormously.

  3xAA - Looks like a dumb PWM regulator. No heatsink, no focus, no top strap for headband.

  You can do an awful lot better on eBay, even for $20.
  Look for XM-L emitter or similar, Li-Ion, metal case for cooling, not plastic.

  (though this is miles better than the other crappy headlamps sold at BCF. I'd be ashamed to sell that landfill.)
