The fifth LED is apparently a red/orange one that tells you when the battery is getting low.
Yea mine too has been blinking the 4th light from saturday morning till sunday night. i just gave up and unplugged it. just tested on my s3 for a minute and it seems to work. havent tested ipad yet though.
I tested charging my phone.
It shows 3 lights when I pull it off the charger, then I plug my phone to it and it shows only 2 lights.
It charged it up to 100% and is still on 2 lights though, so hopefully it's just a buggy lighting thing and not an indication that the battery is not holding charge.
I guess that's because of the voltage drop when charging.

regarding the powerbank, is the AC input 120/240V?

That's what happened with mine too. I am guessing it's normal as it seems to be powering my USB devices just fine, but I'll report here if I have any issues.
