This Polish indie game appears to be at an ATL on Steam, GOG and the PlayStation Store.
Note that the game was added to the PS Plus Extra and Premium catalogue back in September 2024, so if you're subscribed you already have access to it.

The game has extra cosmetic DLCs but it's not necessary to buy them. The devs have made available all story content to all owners of the game, so there's no faffing about with needing to buy an Ultimate edition or any of that nonsense. You don't need the Premium Edition unless you want to support the devs by buying cosmetic addons/soundtrack/artbook.

Generally runs OK on Steam Deck (40-45fps) but it does slow down to 20-30fps in indoor areas.
A sequel called Chernobylite 2: Exclusion Zone is due to be released sometime in 2025.
