I've applied for the AU affiliate but got knocked back; it's handled by a third party here (can't remember who off the top of my head). :) The US and UK ones are a simple form and seemed to be automatic.
You've got to make sure your website/app/etc is all set up and ready to go before they'll approve you. I had a full blog with reviews of apps for kids and they knocked me back as there wasn't enough content (only about 30 posts at that time)
