    Obviously most is markup; but from where?

    Well, there's a sort of international database owned by whomever designed the standard (so, Microsoft, for microsoft certs as an example), which needs to be maintained, and staff who need to provide assistance to employers or such when they request details.

    Im absolutely certain its many fold too expensive to cover costs and a profit; but hey, they invented the 'standard' so if you want it, pay them.

    We do the same with the education department though taxes and such.

    The 'scammy' part, is when you find the same test at hugely differing costs; then you know someone is riding well above the cert 'rrp' cost.
