Yes, I got CBA to do it for me once. Depends on the conditions though. In my case, it was an e-commerce seller never shipping the goods.
Definitely debit and not credit? It is rare to see an online store give their bank details for bank transfers instead of just having an online portal for credit card payments

Just ring your bank and advise them you would like to do a chargeback. They will walk you through the process.

it depends on type of transaction. though its debit card, online transactions are usually classified as credit and you will be able to do a reversal.
Successful with Macquarie Platinum Debit MasterCard on a couple of occasions. e.g. local purchase refused refund although not fit for purpose under ACL and overseas purchase that never arrived. Pre-condition was that the transaction was processed via MasterCard as a ‘credit’ transaction, not via EFTPOS as a ‘debit’ transaction. So depends how your transaction was processed and what bank you are with.

How I am supposed to know being a seller? I always received payment via Paypal, so no idea whether the customer used credit/debit card.
I had a successful chargeback with my Citi debit card a few years back while overseas. Was in dispute with a bus company that never provided me with a ticket. Just had to go through the long process, effectively drawn out over the 45 days or whatever it was.
On a different note, also had ING refund my debit card for an ATM transaction where the cash was never actually dispensed from the machine.
No, very unlikely unless you got hacked (your account used without your permission cause your password was stolen or something). Possibly if you mistyped an account number (but probably not unless the recipient agrees to have the money returned).
