This is dodgy … I tried unsuccessfully to enter using both Firefox and IE11 … completing details and hitting submit doesn't work … the links provided above are just a way of getting you to forward it on

Thanks gnv9! I don't know if it's just me, but I had a lot of trouble trying to submit the form using Chrome on MacOS. Switched to Safari and it worked. Just in case anyone else has issues.
