Today's "The Current" covered Faulty Medical Devices (from Hip-Replacement parts affected by outside temperatures to devices implanted in incontinent women that cause pain,as they come-apart, & continually release parts, via vagina,- thread by thread, over time, ie, until patient can find a surgeon willing to remove the faulty device.
After a year or 2 of Freedom of Info "negotiatioms" CBC's "Data Journo" managed to get Canada's health (& device)regulator - Health Canada - to release their full list of reports of unsatisfactory devices, w/ severity from Minimal to Death:
