Has anyone managed to get form up using 64-bit Windows 10 and IE11?

I just get a blank page. Tried different browser emulation tools and still not working.

Yes, but only after installing the Adobe Form Control plug-in. Note the system requirements say

not compatible with Apple Mac and Microsoft Surface Pro

After more Googling, the answer to the problem is… drum roll… resetting IE11.
