It looks like MNF is charging a $20 BYOD setup fee that includes $10 free calling credits.

$5/year on MNF is probably one of the cheapest did costs if that's all you're after. there are other providers who include the did if you get a calling plan, all depends on what you're actually trying to achieve
There's some recent suggestions here. I'm using SIPTalk (Owned by Telecube) and they've been great, but unfortunately their WP offer is no longer available.

DID is a virtual local phone number that can be used anywhere in the world via the internet using Voice Over IP(VOIP) software or ATA device. For an example if you have a Sydney local number you could be in London UK and be able receive and make calls coming to a Sydney number simply by connecting your device or software to the internet (callers will be paying local rate).
Alternatively you could use this as a cheap landline home phone in Australia ($5 year if you only receive calls)

Yes, that's whole point. You only need data (either mobile or fixed line)
Cut the cord. You can even port your number (if you still happen to have a land line phone)
I am with mynetfone. Cheapest $10 per month. No problems yet after 12 month. Australian phone had a longer lag between dialing and ringing which was a problem for me.
