Opra house, visit bondi and manly beach. Can take the ferry to manly with great views of habour bridge. Dine out in darling harbour or the rocks. Visit the blue mountains as a day trip.
Blue Mountains isn't possible in a wheel chair (I think). You can visit the top but you only see one view and an obstructed view of the Three Sisters. If it were close to the city I think it would be reasonable to recommend it but it's quite out of the way and I'm not sure if the travel is really worth it.
Other stuff is good
around circular quay
Try to avoid going to the aquarium on a weekend. From memory the aquarium walkways are narrow and it gets very crowded on weekends.
Take a picnic and get lost in the Botanic Gardens. There are paved walkways throughout. I never tire of its beauty or the view.
Check out the art galleries, all should be accessible.

if you catch the ferry from Circular Key to Taronga then you'll get a good view of the harbour bridge and opera house.
If it is school holidays, the weekend or a very hot day avoid the sea life aquarium
Mrs macquaries point is a good spot to get both the opera house and the harbour bridge in the same frame
Visit Bondi (train to bondi junction, bus to the beach), have lunch there or something
Also nice ferry ride (Rivercat) Parramatta <-> Circular Quay

We are coming from Perth 26 years old and in November
you can get all the touristy stuff from brochures and Google searches.
If you have the time, and in that way inclined, blue mountains may be an option

Does she get Taxi subsidy? Contact the WA TUS and ask for interstate vouchers as eastern states use different vouchers. They did a year and bit ago, don't know if NDIS BS crap has changed things.
She works full time so I would think not
pretty sure you can get a entry to 3 attractions for one price, but you need to see them in a day.
