That form is a hot mess. It wouldnt let me pick a year prior to 1988 and then wanted my time of birth? Jeeze, I mean I was there but don't really remember the exact time. Saturday evening I believe.
Same thing happening to me. Cant submit form because the phone number is not the "right format". Also it's so weird that they are asking for the time of birth. Lol. Made me laugh. How is that even relevant.

For some reason, my phone number is said to be in the 'incorrect format' I've done it both with a 0 at the front and the +61. Im so confused. Both are apparently wrong.
