Bloodborne GOTY Is The DLC on Disc or Not?

For anybody that have ever bought Bloodborne GOTY and Witcher 3 GOTY retail are the DLC on disc or do they just give you voucher codes to redeem from PSN?

    All the DLC is on the disc for Witcher 3 GOTY, there are no codes.

    Not sure about Bloodborne GOTY, sorry.

    Both games have their DLC on disc - no codes.
