Thought the same but the form has a 'business name' section so might be customised to your own?
I would assume that the 'Business Name' section is just for their reference as it's not mandatory to fill out that section. From what I can see from their site, I'm sure they will send out either their own business cards or a blank generic one, however not a custom one?

Sample business cards are a waste of resources and money, they have no use or benefit, unless you were planning to buy their product.
I'm all for freebies, but this is one of the most pointless I've seen.
