For a couple earning ~190k per year private health insurance is needed to avoid MLS. But my question is basic cover good enough or extra (eg optical, dental etc) is also needed?

It is clear from http://www.privatehealth.gov.au/healthinsurance/whatiscovere… that "private health cover" is distinguished from extras cover.
In short, you don't need extras cover to avoid MLS.
To be exempt from the surcharge, your hospital cover must be held with a registered health fund and cover some or all of the fees and charges for a stay in hospital, with a maximum payable excess per year that is no greater than $500 for singles or $1,000 for couples/families (i.e. if multiple hospital claims are made in a single year, the excess paid by you cannot exceed $500/$1000).
I would buy the extras, get a good medical laywer and be circumspect about anything done! It can be a bit rockstar and roadie
Simple answer , basic cover is sufficient.
