Philips Kitchen Appliance Cash Back from April/May/June 2016 ?

I bought a kitchen appliance to take advantage of the Philips Cash Back offer earlier this year (April~June):

I sent back the necessary form back in May. It's July now and I haven't got the Cash Back.

Has anyone else completed and sent back the forms? If so, have you received your Cash Back?

I have contacted Philips via their live chat on their website today, as well as sent an email asking them to clarify the situation.

They gave me a Sydney phone number, but I don't want to spend lots of money on interstate phone calls (I don't have an unlimited call plan).
