With a 0% for 12 months balance transfer/cheque-to-self promotion, is your annual date the card application date or the date on which the balance transfer was initiated (date on your first statement)?

Have you had a pdf statement since you did it? If so I think there is a list on the statement of your interest rates.
Have done a couple BT with citi and its always been Application Approval date (as in when your card was approved) on my accounts.
Just call them, best is twice, to speak with different staffs just to make sure the exact date when you need to pay off.
It's depends on you billing cycle so most people don't need to pay off til 12.5 months or more.
