visit the promotional website of the Participating Venue where the purchase of the Participating Product was made:

for Sip N Save venues, the promotional website is www.sipnsave.com.au/jacks-birthday-2015; and
complete and submit the online form on the promotional website including personal details (first name, last name, email, phone, address and date of birth) and the unique code found on the back of the entry form provided at the time of purchase.
The entrant will be entered into Draw 1 or Draw 2, depending on when they entered the Promotion. The entrant must enter during:
