My son has an old iphone 4 which he has just fixed up (new screen) it was originally on a Optus plan about 4 years ago, and has not been used for at least a year. Can he unlock it from Optus?

Then it'd be locked to Optus.
Optus phones on plans are not locked
"Update your device to iOS 7 via iTunes. It's that simple. Once the update is complete your iPhone will be unlocked."
This is following Optus auto-unlocking all iPhone handsets on 11 September 2013. If the phone is not iOS 7 or above, it will still be locked.
He put a telstra sim in it and it said carrier optus?
Connect it to itunes it may change the carrier settings, also sometimes when you reboot with the new sim it will update the carrier settings for you
Easily unlockable. Just go to any Optus Branch.
EDIT: As above, it's much easier than that. Just update to iOS 7, and the auto-unlock will be applied.
JailBreak.. as your phone already might be out of warranty.

Should be unlocked already from Optus, have you tried a different carrier sim.
