I'd buy a puppy carrying crate as you'll probably need one for going to the vet etc anyway and wrap or put a bow around that (leave the door exposed). Then get him to close his eyes and put his hand inside?
Spotlight. Reject Shop. Add a ribbon and bow yourself, you can buy the bows pre-tied. They have patterned ones and plain ones, cardboard and paper-mache used for hat boxes etc.
