Help with Getting Rid of Margin/Sticky Notes on Legal Document! Urgent!

Hey
I need to get access to an enduring power of attorney form

However i can't seem to get rid of the sticky notes attached, the questions marks on the left of the pages.

It would an big help if someone could get of them for me and link me a download without the sticky notes. I need to also be able to write in the specified areas.

    The trick seems to be to first save the document somewhere on your hard-disk. (Right-click on it, and select "Save As" in Google Chrome.)

    Then open it up with Adobe Reader. (My version is Adobe Reader XI v.11.0.10)

    The question marks will now disappear when you print it.

    Different ways for different OS's

    If its on a Mac, download file and open with Preview. Then select from the File menu "duplicate", you can then click on the question marks and delete them

    Then save the duplicate after you delete the question marks. When you do this the Save As, Print and Reset Form buttons also go.
