Actually this looks like a new one, probably to look for improvements. They asked for more details this time. Also last time only the adult Opal was available. You might have better chance if you have a less common card type. Be aware that somebody collating the results can see all your Opal activity as they ask for your card number. Me I don't care I'm just a commuter.

I got to participate in 2 surveys. One in Jan 2014 and one in June 2014.
I would say they'll probably do more surveys in the next few months to evaluate the Opal system after the recent roll outs to child and concession cards.
