I know Macca's do this but I was wondering what's the policy for other places (KFC, HJs, Grilld, etc)

Thanks, and I figured that there would be some kind of weirdness with HJs because of the free refills since at my local HJs the staff just give you a cup to use if you order a soft drink so if I would think that if I ordered a water they would give me a cup and just keep an eye on me or not give me a cup at all.
BTW how do the staff feel about using the $1 frozen coke cups for refills?
There's a sign at the HJ's I was at the other day stating that they weren't to be used for refills. I'm not sure if they're widespread or not.
As far as i am aware, most establishments provide amenities that would surely have a tap. The use of reusable water bottles is a neat way of storing rain or tap water. If you eat hamburgers etc i am sure water out of a trough would be fine..
If you eat HJ/Mcd hamburgers etc I am sure water out of a toilet would be fine.

This isn't considered a fast food place but Coffee Club offered free water.
I work for HJ's. I'm unsure of the nationwide policy (I'll look it up next time I'm at work for you), but I'm more than happy to do a cup of water for you for no charge.
If you take advantage and try and get free refills of softdrink with that same cup, chances are the staff won't appreciate it.
