I know you can get a free sub at Subway, and a free smoothie at Boost… but are there any others?

Just FYI they have now changed this. You will need to have made a spending with them (and have the card scanned, obviously) within the last 6 months prior to your birthday month in order to qualify, and you will be sent an email with unique redemption code on the start of your birthday month to be used for that month. It appears that they are doing the email rounds manually too, so one per person only :)
