I had the same problem. Wouldn;t work at all last night, but seems to be fixed now. Toothpaste is hopefully on its way!
I have no problem the first time I tried. These small toothpaste samples are very useful for travelling because of their size.
However I won't be surprised that all the promotional stuff are out-sourced to PR firms, who then out-source the development of website to the development house that bids the lowest…

You'd think that Colgate could employ a professional programmer! Took me a few tries before the site would accept my details. I had to put in a second address line though not required. My wife did not succeed at all - tried IE as well as Firefox. There is not even an email address on the page to complain. Makes you wonder who they employ to mix the toothpaste!
